As the "system" exists at PSD now, you do not know what the discrepancy was, who wrote it up, and who fixed it (if it was fixed) (as nothing is documented as required)...

...See if you can find one of these (return the job to shop pickups because it has too many defects upon initial inspection) written recently. Our inspection system has been corrupted to such a point, that an inspector who wrote a "return to shop" pickup would probably receive a "return to unemployment" "pickup" shortly thereafter.
